ABSTRACT

SUMMARY: Clear awareness of the vascular variations is critical in surgeries, which may cause massive hemorrhage during surgical procedures. During educational dissection of a male cadaver, we encountered a combined variation of the left obturator artery and ipsilateral aberrant inferior epigastric artery. The left obturator artery originated from the external iliac artery, then coursed inward, adherent to the superior pubic ramus. The left inferior epigastric artery originated from the femoral artery, and coursed behind the femoral vein. These anatomical variations shown in one person were extremely rare. This is particularly true with regard to these variations while performing pelvic and inguinal region surgeries.


El conocimiento claro de las variaciones vasculares es fundamental en las cirugías, ya que pueden causar una hemorragia masiva durante los procedimientos quirúrgicos. Durante la disección educativa de un cadáver de sexo masculino, encontramos una variación combinada de la arteria obturatriz izquierda y la arteria epigástrica inferior ipsilateral aberrante. La arteria obturatriz izquierda se originaba en la arteria ilíaca externa, luego discurrió hacia medial, adhiriéndose a la rama púbica superior. La arteria epigástrica inferior izquierda se originaba en la arteria femoral y discurría por detrás de la vena femoral. Estas variaciones anatómicas mostradas en una sola persona son extremadamente raras. Esto es importante de conocer estas variaciones cuando se realizan cirugías de las regiones pélvica e inguinal.

ABSTRACT

La anatomía de la región inguinal está bien descrita y detallada en la literatura, pero existen algunas controversias vinculadas al ligamento reflejo y a su inserción medial. El objetivo fue analizar la inserción del ligamento inguinal reflejo del músculo oblicuo externo en la línea alba, mediante lo observado en disecciones cadavéricas. Se utilizaron 30 cadáveres formolizados, en los que el ligamento reflejo fue disecado hasta su inserción en la línea alba. Se encontró la presencia del ligamento inguinal reflejo en 25 (83,33%) casos, 16 (64%) hombres y 9 (36%) mujeres, de los cuales sólo en 2 (8%) casos se observó la inserción interdigitada del ligamento inguinal reflejo con el contralateral; y en 5 (16,67%) casos (3 mujeres y 2 hombres), se constató ausencia del mencionado ligamento. El ligamento reflejo fue identificado en la mayoría de nuestros ejemplares. Este ligamento contribuye a la formación de la pared posteromedial del anillo inguinal superficial, siendo importante la necesidad de su conocimiento anatómico para su aplicación en la reparación quirúrgica de hernias inguinales con colocación protésica por vía anterior.


Anatomy of the inguinal region is well described in literature, but there is lack of information related to the reflected ligament and a dichotomy regarding its medial insertion. The aim was to carry out an observational analysis, through dissections, of the insertion of the reflected ligament of the external oblique muscle at the linea alba. We used 30 formolized corpses were, on which the reflected ligament was dissected to its insertion into the linea alba. Of the 30 bodies, we found the reflected ligament in 25 (83.33%), being 16 male (64%) and 9 females (36%), but only in 2 of them (8%) its insertion interdigitated with the contralateral. In 5 (16.67%) cases, 3 female (60%) and 2 male (40%), the absence of that ligament was found. The reflected ligament was identified in most of our specimens. Being part of the posterior limit of the superficial inguinal ring, its anatomical knowledge is important for the surgical repair of inguinal hernias with prosthetic placement, using anterior access.

ABSTRACT

Right Inguinal region pain is a common complaint evaluated in surgical department. The number of differential diagnoses is lowered when the pain in a male patient is associated with a palpable tender mass. These diagnoses include inguinal hernia, inflamed inguinal lymph node, rectus sheath hematoma, cryptorchidism, mass derived from the spermatic cord, and polyorchidism. Right Inguinal region mass and pain caused by a spermatocele are unusual. Here we report a case of extra scrotal spermatocele causing right Inguinal region swelling and pain. To our knowledge this is a second reported case.

ABSTRACT

O carcinoma de células escamosas (CCE) é uma das neoplasias mais frequentemente observadas no trato genital de cães. O tratamento de eleição para o CCE é a exérese com ampla margem cirúrgica (1 a 3 cm), porém quando a região prepucial está comprometida, se torna difícil obter margem adequada, tanto pela anatomia da região quanto pelas características agressivas dessa neoplasia. A penectomia com uretrostomia pré-escrotal seria a técnica mais indicada nesses casos, entretanto, complicações pós-cirúrgicas podem reduzir a qualidade e a expectativa de vida do animal. O objetivo deste relato foi demonstrar uma técnica cirúrgica conservativa, com base em plásticas reconstrutivas com retalhos cutâneos, como tratamento para CCE em prepúcio e região inguinal, associada ao uso do piroxicam como terapia adjuvante. O animal não apresentou recidiva do tumor ou complicações secundárias ao tratamento até a última avaliação, um ano após o término do tratamento.


Squamous cell carcinoma (SCC) is one of the most frequently neoplasms observed in genital tract in dogs. The treatment of choice for the CCE is the excision with wide surgical margin (1-3 cm), but when the foreskin region is committed becomes difficult to obtain adequate margin, due both the anatomy of the region and the aggressive characteristics of this neoplasm. The penectomy with pre-scrotal urethrostomy would be the most suitable technique in such cases, however, postoperative complications may reduce the quality and expectancy of life. The objective of this report was to present a conservative surgical technique, based on reconstructive plastic with skin flap as a treatment for SCC in prepuce and inguinal region, associated with the use of piroxicam as adjuvant therapy. The dog was evaluated during one year after the end of treatment and haven't showed any tumor recurrence or secondary complications to treatment.

Introducción: el objetivo de este trabajo es presentar un caso de empalamiento a través de la región inguinal. Las heridas por empalamiento son infrecuentes y, en ocasiones, de extrema gravedad, necesitan de una actuación rápida del personal médico de urgencias y del cirujano. Caso clínico: se presenta el caso de un varón de 40 años que sufrió un accidente laboral con empalamiento de un hierro de ferralla a través de la región inguinal derecha, el cuerpo extraño penetró en la cavidad abdominal. Se expone ampliamente el caso clínico, así como los procedimientos realizados en el diagnóstico y el tratamiento de este tipo de lesiones. Conclusiones: las heridas por empalamiento son infrecuentes y suponen un reto para el personal médico que atiende al afectado desde el primer momento, tanto por lo complejas que pueden ser, como por la necesidad de una actuación rápida, sin poder conocer a priori, en muchas de las situaciones, la extensión verdadera de las lesiones, que se evidenciará durante el posible acto operatorio(AU)


Introduction: the objective of this paper was to present a case of impalement through the inguinal region. The impalement injuries are infrequent and sometimes extremely serious. These injuries require prompt action of the emergency medical personnel and surgeon. Clinical case: a forty-year old man, who had an occupational accident resulting in impalement of an iron rebar through the right inguinal region and penetrating abdominal cavity. The clinical case and the procedures performed in the diagnosis and treatment of these injuries were presented in detail. Conclusions: The impalement injuries are rare and represent a challenge to the medical staff that treat the patient from the very beginning, because they can be very complex and require fast action and treatment. In many cases, the real dimension and severity of lesions at first is unknown and can only be assessed during surgery(AU)

ABSTRACT

A tumoral calcinosis is considered a rare disorder with a poorly understood etiology. This uncommon soft tissue calcification is characterized clinically by the presence of an irregular, painless, periarticular soft tissue calcifying mass. People from African decent are mainly affected with the disease, which typically occurs in the periarticular regions. We report a case of a middle aged woman who complained a left inguinal subcutaneous mass. The characteristic clinical features, radiological and pathological findings are described. A diagnosis of a tumoral calcinosis can be difficult to make due to the unfailiarity of its clinical and radiological features. This presented report will help surgeons to easily diagnose this condition.
